Okay, I submitted the character sheet (it should be Public in case anyone beyond the GM wants to look at it). I'll try and doodle up a visual representation for him a bit later. There wasn't a space for the Background, so I'll just post it here for now:
Background
Zachary Toomes was one of the many lost souls who roam the world without purpose. His father was a veteran and a drunk, and his mother was disloyal and blamed her lot in life on Zach's birth. With such a home life, you might imagine that Zach spent a great deal of time looking for excuses not to go home. His friends were similarly disaffected youths, who stayed out late, roaming the town's graveyard, and looking for a higher purpose. A purpose they believed they found when the youngest among them followed their dreams to an abandoned church, which apparently had been built upon the site of another, much older place of worship.
Zach wasn't much of a believer himself, but if his friends wanted to dabble in occult rituals that promised to give them everything they believed they deserved, who was he to stand in their way? He was still rolling his eyes beneath his polyester-mix hood when his own friend plunged the knife into his chest. It turns out, the others had conspired together -- the ritual required the "blood of an unbeliever" and how better to seal the pact between the survivors than to remove the only one who knew enough to endanger them? He was buried in a shallow grave on the outskirts of town, where, supposedly, he would never be found.
Deep within the embrace of the earth, however, arcane forces continued to twist and form. The ritual may have been performed correctly, but those responsible were not of the bloodline of priests -- the nameless god of that place was not resurrected. Instead, a shard of its ineffable power clawed at Zachary, twisting his body into a creature which would pursue the temple robbers and take back the powers they had stolen. Only when he had done so would he know rest ... but Zach doesn't particularly trust his "patron" to follow through on his end of the deal in the end.
